All full-time, regular employees who are scheduled to work a minimum of 40 hours per week are eligible for health (medical and vision), dental, life and disability insurance coverage the first day of the month following employment. Eligible dependents (including spouse and/or eligible, dependent children, or domestic partners) may also be enrolled at the same time. Temporary employees are not eligible for insurance coverage.
Group Health Options provides comprehensive health-care benefits. Participants have the option of choosing a plan that allows them to use facilities and health care providers that have been designated by Group Health or choose their own out of the network providers.
Vision Care Services are also provided by Group Health. One routine eye exam per year (in network) is covered as well as $150 towards glasses/contacts in a 24-month period.
Dental Coverage is provided through a separate insurance carrier (Washington Dental Service). The dental plan provides for preventative care, as well as basic and major restorative services. A limited lifetime benefit is also provided for orthodontia services. Financial incentives are provided to encourage regular dental checkups and related dental treatment.

DENTAL INSURANCE COVERAGE
Dental Coverage is provided through Washington Dental Service. The dental plan provides for preventative care, as well as basic and major restorative services. A limited lifetime benefit is also provided for orthodontia services. Financial incentives are provided to encourage regular dental checkups and related dental treatment.
LIFE INSURANCE COVERAGE
Basic Life, Accidental Death & Dismemberment (AD&D) insurance is provided for all eligible employees at no cost.
Supplemental Life insurance can be purchased for employee, spouse and children. Employees pay the full cost for any supplemental life insurance.
All coverage is subject to approval by Servatron's insurance carrier.
DISABILITY COVERAGE
Short-Term Disability (STD) insurance provides eligible employees with partial income protection for disabilities that extend beyond 14 days up to day 90 at no cost.
Long-Term Disability (LTD) insurance provides eligible and enrolled employees with partial income protection for disabilities that extend beyond STD benefits at highly subsidized rates.
EMPLOYEE ASSISTANCE PROGRAM
This program, provided to all eligible employees by APS Healthcare, allows short-term, confidential counseling (personal, financial, legal) for you and your household at no out-of-pocket expense to you.
401(k) INCENTIVE SAVINGS PLAN - 401(K)
Servatron allows all regular, eligible employees to participate in a 401(k) plan through
T. Rowe Price. The company maintains a discretionary matching program.
PROFIT SHARING
All employees, including temporary personnel, who satisfy a three month service requirement, may be eligible to receive a semi-annual profit sharing distribution. This profit sharing is based on the company meeting specific financial targets and successful accomplishment of corporate profit goals.
FLEXIBLE BENEFITS PLAN
The purpose of this plan is to allow eligible employees to use funds, provided through employee salary reduction, for medical and dependent care reimbursement. The funds enable the employee to pay for the selected coverage with pre-tax dollars, which reduces the employee’s taxable income.
PAID TIME OFF (PTO)
Full-time, regular employees will accrue hours for Paid Time Off (PTO) to be used for vacations, sick time, personal injury, doctor appointments, daycare issues, school and other personal activities:

Servatron currently provides 72 hours of holiday pay.
